IMC clinic — an appointment form

IMC is a family clinic in Almaty. Patients are seen in person and over video.

In 2022 Rocket Firm launched the clinic’s landing page, but instead of a regular request form we designed a full booking form.

What I did as lead designer:

  • Designed a three-step booking form: doctors, time and contacts.
  • Removed interface friction: by default the nearest doctor and today’s date, so the patient only picks a visit time.
  • Separated appointment addresses at different branches with colour indicators.
  • Adapted the design for mobile and built a component base for development.

Data first, filters later

A common scenario: the patient isn’t going to a specific doctor, just to a clinic they trust. For this case the form is set up by default: the nearest available doctor for today, so all that’s left is to pick a time and leave contacts.

Booking with a specific doctor

The second most common scenario: the patient wants to book with their own doctor. Here the first step is choosing the doctor and seeing exactly which branch they work at and when. For these cases I designed the states and indicators the form needs.

Adapting for mobile screens

Up to 80% of all bookings were made from phones, so the adaptive screens were designed with close attention to detail.
